We are currently able to accept new patients. The area served by the practice includes Mold, Gwernymynydd, Cadole, Maeshafn, Gwernaffield Pantymwyn, Cilcain, Rhydymwyn, Sychdyn, Nercwys, Bryn y Baal, Mynydd Isa from the Pren Hill to Mold and Northop, but only as far as the A55.
New patients are welcome. Please attend the reception desk and ask for a registration form. Most people take this home to complete and return with the form and ID for identification purposes. We will require one form of ID and one proof of address.
Patients coming from abroad and wishing to register permanently for the first time will need to provide appropriate documentation e.g., work permit, passport, entry visa etc.
